  • Uniform Fitting & Shopping

    One of the first steps in preparing for the school year is purchasing your student's uniform. Families are encouraged to schedule a uniform shopping appointment at the Flynn O'Hara store in Plainview as soon as possible.

  • State Requirements: Physical Examination, Immunizations and Dental

    New York State Department of Education and New York State Department of Health Regulations. 


     **Students in ALL grade levels, who are trying out for an athletic team, must have a current physical on file.  NO EXCEPTIONS!**


    Physical examinations for school are to be documented on the NYS Required Health Examination(PDF).


    State Requirements: Physical Examination, Immunizations and Dental

    Physical examinations for school are to be documented on the NYS Required Health Examination. ONLY the approved form will be accepted by schools for health examinations conducted on or after 1/31/2021. The updated form has a checkbox for your child’s pediatrician to mark that they have reviewed “Family Cardiac History”. This is in reference to the Dominic Murray Sudden Cardiac Arrest Prevent Act.      As per New York State Education Law:


    A health examination (physical) is required for all students entering the school district for the first time and when entering Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, 9th, and 11th grade. The health examination (physical) must be handed added to Appryse before the first day of school.

    •  The exam must be dated within 1 year of the start of school.
    • At the time of your visit, please have your doctor complete the NYS Required Health Examination form, sign and date it. A computer print-out of your child’s immunization record is also required. 
    • New York State Law requires that your child’s BMI (Body Mass Index) and BMI % Percentile be included in their physical. Please make sure that your child's doctor documents this information on the green card or includes this in the print-out.
    • New York State also requests a dental exam (dated within 1 year of the start of school) for all students in grades Pre-K, K, 1, 3, 5, 7, 9, 11, and new to school. A Dental Health Report (white form) must be signed and dated by your child's dentist and submitted to the Medical Office before the first day of school. 

    Students who do not provide proof that they are up to date with the required vaccinations will be excluded from school as per NYS guidelines. Please see below:

     

    Per the NYSDOH School Assessment and Compliance Unit:

    Students missing vaccine doses for any reason must comply with Public Health Law Section 2164 requirements. Children entering or attending school in New York State, including summer school and distance learning, must comply with said requirements.

    HIGH SCHOOL IMMUNIZATION REQUIREMENTS: 

    • Diptheria and Tetanus (DtaP/DTP) - 3 doses
    • Tdap - 1 dose
    • Polio (IPV/OPV) - 4 doses 
    • Measles, Mumps and Rubella (MMR) - 2 doses
    • Hepatitis B - 3 doses
    • Varicella - 2 doses (1 dose if first dose was received at age 16 or older)
    • Meningococcal conjugate (MenACWY, Menactra, Menveo) - 2 doses (first dose in 7th grade) and a second dose in the 12th grade (second dose must be after the age of 16).

    Please remember that your child will be excluded from school if New York State Laws are not met.


    Appryse

    Instructions for enrolling in Appryse using Google Chrome:

    1. Enroll for academic year 2026-2027. If your daughter is a returning student, you will select the “I have an Appryse Account” option.
    2. Receive acceptance from school. You will be notified via email if your enrollment was declined.
    3. For new students, download the physical examination form by clicking on the “Download Blank Form” button. If you enroll more than one daughter, each one will have a “Download Blank Form” button.
    4. If your daughter is a returning student, her current physical on file will be rolled over after you re-enroll.**
    5. Obtain immunization documentation and, if needed, complete and sign the Sacred Heart Academy’s medication forms.
    6. After completion, all forms must be uploaded together as one PDF file to Appryse by clicking on the “Upload Completed Form” button.
    7. Receive acceptance from the school nurse provided all medical documents are sufficient. You will be notified via email if your information provided is not acceptable.

  • Class of 2030 Orientation

    August 31, September 1, and September 2

    8 a.m.- 12 p.m. 

    9th-grade students


    Students are asked to dress comfortably and wear sneakers.  Please, NO flip-flops or shoes without backs, as they may pose a safety hazard. Students will enter through the Theatre entrance on Cathedral Avenue. 


    Drop off and pick up will be on Cathedral Avenue.


    Lunch will be provided on Days 1 and 2.  Please have your daughter bring her lunch and a drink for Day 3.


    Monday, August 31 and Tuesday, September 1

    • Meet Homeroom teachers
    • Participate in small-group icebreakers to get to know one another
    • Run through Day 1 schedule 
    • Receive Chromebooks and SHA email addresses
    • Meet their Deans and Counselors 
    • Review Tips for Summer Reading Test
    • "Passport Tour" to explore the building with the help of their upperclassmates
    • Participate in stress-relieving activities 
    • Eat lunch as a class

    Wednesday, September 2: On Campus Retreat Day

    • Familiarize students with the CSJ Mission, including a Gallery Walk to "meet" the Founding Sisters
    • Participate in an Arts and Crafts Project and a time for Reflection
    • Introduction to Serving Others and Community.  All SHA students are required to complete at least 20 hours of community service per year.
    • Participate in their first Service Project as a class

    Parent Orientation & BBQ

    Tuesday, September 1 at Sacred Heart Academy

    • 6 PM: Meet the Administrators Night / 9th Grade Parent Orientation.
    • 7 PM: 9th Grade Parent BBQ (Parents only). Please RSVP for the BBQ.

    ADDITIONAL INFORMATION: 

    Drop-off and pick-up will take place in the front of the building, along Cathedral Avenue.

    Students will enter through the Theatre entrance on Cathedral Avenue.To avoid traffic congestion, consider designating a pick-up spot further down Cathedral Avenue, closer to Four Sisters Field. Note, the school driveway will be closed to vehicle traffic to ensure safety, as the rear of the building is an active pedestrian zone.

    Lunch will be provided on Monday and Tuesday.  Please have your daughter bring a packed lunch and drink on Wednesday (Day 3).

    The students are asked to dress comfortably and wear sneakers.  Please, NO flip-flops or backless shoes, as they may pose a safety hazard. 

    Orientation does not conflict with Sports Tryouts.


  • Transfer Orientation

    September 1, 2026

    8 AM - 12 PM


    The program is designed to foster a sense of belonging and sisterhood among the transfer students and to give them practical information regarding their schedules, the school building and to meet members of our administration. 


    Below is an outline of the various activities and topics we will cover, as well as some additional information regarding pick up/drop off, and dress code.
