Transcript Request Form

Please complete the form below completely to request a copy of your transcript from Sacred Heart Academy.

This method of requesting a transcript is only for students who are graduates of Sacred Heart. Current students should contact their Guidance Counselor for transcript requests.

There is a $5 fee for transcript requests (graduates of the class year 2023 are not required to pay this fee). After completing the form, please mail a check to the attention of:

Please note that transcripts that are faxed will not include a seal and are considered "unofficial" transcripts. "Official" transcripts must be mailed.
